Our team-based approach includes:

  • Social Workers - providing emotional support and acting as advocates for the resident and liaison to their family
  • Physical, Occupational & Speech Therapists - conducting assessments and monitoring the program results
  • Pastoral Care - a full-time Chaplain provides spiritual and emotional guidance through Chapel services, and on a personal level when comfort is needed
  • Therapeutic Recreation - offering a variety of activities for the residents on each unit

Dietary - a registered Dietician and Diet Technicians address nutritional concerns and provide meals to meet each resident's needs and preference

Frequently Asked Questions

Does insurance cover the cost of my stay? If not, how much will we pay for it?

Insurance coverage vastly varies from plan to plan. We will confer with your insurance provider to ascertain the coverage limits and any out-of-pocket costs.

If someone does not have insurance, our team can work with you to review private payment/Medicaid options. We also provide free Medicaid application assistance.

How long does the admission process take?

The length of the admission process varies, depending on care needs and where the potential resident is coming from.

If the individual is coming from a hospital, all discharge plans are coordinated through the hospital caseworker. If the individual is coming from the community, the following documentation is needed to process the application:

  • Completed admission application
  • Medical records from the six most-recent medical visits
  • Medication record and history
  • Physical evaluation from a physician

A Patient Review Instrument (PRI) and Screen are required for all admissions. Once all of the information is received, an admission response is typically delivered within 24 hours.

What is the average length of stay?

The length of stay varies for each individual based on the medical diagnosis, therapy needs, and physician recommendations.

Short-term rehabilitation residents will meet with your unit manager, PT, OT, SP, caseworker, and any others involved in your care within two weeks of your stay to talk about next steps.
