As a faith-based organization, Community Wellness Partners is committed to providing quality, diverse, and inclusive spiritual care to our community of residents, participants, their families, and our staff. An integral part of each resident’s care plan is their spiritual health and well-being.

With compassion and a dedication to nurturing the human spirit, our Chaplain, in collaboration with volunteer clergy and lay persons, provides one-on-one, communal, innovative, interactive, and spiritually nurturing programs and services to those in their care. Respect for and honoring of each person’s faith tradition, belief system, and cultural identity are held in the highest regard.

Spiritual care also extends to our staff. Those who dedicate themselves to caring for our residents have access to spiritually focused support, including active listening and advocacy while they are at work. Our Chaplain is trained and committed to being present to support, care for, and walk alongside anyone as they navigate their days. Confidentiality is maintained at all times.

Spiritual wellness in aging is recognized as essential to overall well-being. We strive to honor the aging community’s need for person-centered spiritual care and support in order to foster spiritual well-being throughout the aging process.

What to Expect from Spiritual Care

  • Facilitating weekly worship services, including traditional expressions such as Rosary, Mass, Bible Study, Hymn Sing, and Easter and Christmas services
  • Providing one-on-one spiritual care
  • Offering grief support and emotional comfort
  • Intervening during personal and communal crises
  • Providing spiritual care, counsel, and education
  • Serving as a liaison with churches and other faith-based community organizations
  • Facilitating memorial and funeral services
